Job responsibilities: Use infection prevention practices during patient encounters and procedures. Document care delivered, patient response, and communications in the medical record. Work 40 hours per week with 8-hour shifts. Coordinate scheduling, referrals, and follow-up care with clinical and administrative teams. Support care coordination activities across providers to promote continuity of care.
