Saint Alphonsus Medical Group (SAMG) is a physician led and accountable 650+ provider multispecialty group practice seeking a Family Medicine Practitioner (MD/DO) to work full-time at its Lake Hazel clinic. The Primary Care providers at the Lake Hazel clinic are dedicated to providing services for a full spectrum of patients to include birth to end of life. The 8,000 square foot clinic is surrounded by an underserved residential area that continues to experience growth and development of new housing. The Lake Hazel clinic has procedure rooms, lab and digital X-Ray services on site, in addition to a fully staffed Urgent Care facility open every day 8am - 730pm. This is a full-time role with a traditional clinic work week, M-F; no less than 34 hours of scheduled patient contact time. Practice call shared with other eligible members in the SAMG Department of Family Medicine/Internal Medicine (Currently 1:40). Support: 24/7 CRNA Anesthesia; Level IV Trauma Center; ED; Saint Alphonsus FM clinic on-site; community family medicine clinics; SAHS specialty physicians rotate in outreach, larger system resources. Requirements: Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree Successful completion of an accredited ACGME/AOA Family Medicine or Internal Medicine residency program. Board certification through the American Board of Family Medicine (ABFM), American Osteopathic Board of Family Physicians (AOBFP), or American Board of Internal Medicine (ABIM) (or obtain such certification within 3 years of employment ).
