Job Title: LTSS Service Care Manager Position Overview The LTSS Service Care Manager is responsible for coordinating and managing comprehensive care for members with long-term services and support (LTSS) needs. This role focuses on developing individualized care plans, ensuring service delivery, and improving overall health outcomes through collaboration with members, caregivers, and healthcare providers. Key Responsibilities Manage a caseload of members requiring long-term care services and support Conduct comprehensive assessments to evaluate member needs, support systems, and eligibility for services Develop and implement individualized service care plans in collaboration with members, caregivers, and providers Coordinate and authorize referrals for healthcare and community-based services Monitor service delivery to ensure care plans are executed effectively and without gaps Perform regular member outreach, including monthly and quarterly check-ins (in-person and telephonic) Conduct in-home or community visits (up to 80% travel required) Collaborate with healthcare providers, discharge planners, and interdisciplinary teams to ensure continuity of care Assist members with navigating benefits, services, and resolving complaints or appeals Maintain accurate and timely documentation, including assessments and care management notes Identify opportunities to improve care plans and address deficiencies in service delivery Work Schedule & Environment Monday–Friday, 8:00 AM – 5:00 PM (EST) Combination of remote work and field-based visits 1–2 days remote (administrative work) 3–4 days in the field conducting member visits Travel required within a regional radius; mileage and lodging reimbursed when applicable Occasional in-person team meetings or trainings (quarterly) Initial training period (4 weeks): full attendance required, camera-on virtual sessions Required Qualifications Bachelor’s degree required Active and unrestricted license in one of the following: Registered Nurse (RN) Licensed Clinical Social Worker (LCSW or LCSW-A) Valid driver’s license 2–4+ years of healthcare case management experience Experience in physical health settings such as: Hospital case management Home health Discharge planning Long-term care Preferred Qualifications 4–6+ years of healthcare case management experience Experience in member-facing or community-based care Strong background in care planning and utilization management Familiarity with electronic medical records (EMR/EHR) systems Proficiency in Microsoft Office Suite Required Skills & Competencies Strong care coordination and case management skills Ability to work independently in a remote and field-based environment Excellent communication and interpersonal skills Strong organizational and documentation abilities Tech-savvy with the ability to navigate multiple systems Critical thinking and problem-solving skills Additional Requirements Ability to travel extensively (up to 80% of the time) Must be comfortable with in-person member interactions Must be authorized to work in the United States without sponsorship Assignment Details Contract duration: Approximately 6 months Potential for extension or conversion to full-time
