POSITION OVERVIEW: We are seeking a highly detail-oriented Medical Claim Keys & Insurance Verifier with specific expertise in UB-04 claim forms. The ideal candidate will accurately key hospital and facility claims, verify insurance coverage, and help ensure claims are processed efficiently without errors.

KEY RESPONSIBILITIES

• UB-04 Claim Keying: Accurately input UB-04 claim information into electronic billing systems, including patient demographics, provider data, service codes, and charges.
• Ensure all required UB-04 fields are completed, including: Patient information (Name, DOB, SSN, insurance ID) Admission and discharge dates Type of bill and revenue codes CPT/HCPCS procedure codes and ICD-10 diagnosis codes Total charges, payments, and adjustments
• Review source documents (hospital encounter forms, EHR notes, physician orders) for completeness and accuracy before keying.
• Identify and correct errors in claims prior to submission.
• Communicate with providers, coders, and insurance companies regarding claim discrepancies or missing information.
• Verify insurance eligibility and benefits for inpatient, outpatient, and skilled nursing facility claims.
• Maintain strict confidentiality and compliance with HIPAA regulations

Position Requirements - Qualifications:

• High school diploma or equivalent; Associate's degree in healthcare administration preferred.
• Prior experience in medical claim keying, especially UB-04 institutional claims.
• Knowledge of revenue codes, CPT/HCPCS codes, ICD-10 diagnosis codes, and hospital billing rules.
• Familiarity with electronic medical record (EMR) and practice management systems.
• Exceptional attention to detail, accuracy, and organizational skills.
• Strong written and verbal communication skills.
• Ability to manage multiple claims efficiently and meet deadlines.
