Occupational Therapist Assistant, Home Health

CenterWell Home Health is seeking a compassionate and motivated Occupational Therapist Assistant, Home Health, to provide high-quality rehabilitative care to patients in their homes. In this role, you will work under the direction and supervision of a licensed Occupational Therapist to help patients regain independence, improve safety, and enhance their quality of life in the home setting. You will implement individualized treatment plans, assist patients with therapeutic exercises and functional activities, and provide training in activities of daily living such as bathing, dressing, grooming, home management, and use of adaptive equipment. You will carefully observe patient responses, document progress, and report changes in condition to the supervising therapist and clinical team to support safe, effective care and timely goal attainment. As an integral member of the CenterWell Home Health team, you will collaborate closely with nurses, therapists, physicians, and family caregivers to coordinate care, reinforce education, and promote patient autonomy. You will travel to patients’ residences, assess the safety of the home environment, and offer practical recommendations to reduce fall risk, support energy conservation, and improve functional mobility.
