CenterWell Home Health is seeking a dedicated Speech Language Pathologist, Home Health to deliver high-quality, patient-centered care in the comfort of patients’ homes. In this role, you will provide skilled speech-language pathology services to adult and geriatric patients, helping them improve communication, cognition, and swallowing so they can maintain independence and a higher quality of life at home.Working within the Healthcare & Medical Services department, you will conduct comprehensive assessments, develop and implement individualized plan-of-care goals, and provide evidence-based therapeutic interventions tailored to each patient’s medical condition, functional status, and personal goals. You will collaborate closely with an interdisciplinary care team—including nursing, physical therapy, occupational therapy, social work, and physicians—to ensure coordinated, safe, and effective care. Documentation of assessments, progress notes, and care plans will be completed accurately and timely in the electronic health record, in accordance with regulatory and CenterWell standards.Your responsibilities will include evaluating speech, language, cognitive-communication, voice, and swallowing disorders; establishing measurable treatment objectives; and regularly reassessing and adjusting plans based on patient progress and physician orders. You will educate patients and caregivers on home exercise programs, compensatory strategies, safety techniques, and use of adaptive equipment, promoting autonomy and empowering families to support ongoing rehabilitation. You will also participate in case conferences and quality-improvement initiatives, demonstrating CenterWell’s core values of compassion, integrity, and excellence.This position is ideal for a Speech Language Pathologist who values one-on-one patient interaction, enjoys working independently in the field, and appreciates the flexibility and impact of home health practice.
