CenterWell Home Health is seeking a compassionate and skilled Speech Therapist to provide one-on-one, in-home speech-language pathology services to adult and geriatric patients. As a Speech Therapist with CenterWell, you will deliver evidence-based evaluation and treatment to patients in their homes, helping them improve communication, cognition, and swallowing so they can live as independently and safely as possible. You will conduct comprehensive assessments, develop and implement individualized plans of care, and collaborate closely with physicians, nurses, occupational and physical therapists, and family caregivers to ensure coordinated, high-quality care. In this role, you will travel to patients’ residences to perform speech, language, voice, cognitive-communication, and dysphagia evaluations, establish measurable goals, and provide ongoing therapeutic interventions tailored to each patient’s needs, health status, and home environment. You will educate patients and caregivers on home exercise programs, safe swallowing strategies, and communication techniques, documenting all care accurately and timely in the electronic medical record in accordance with regulatory and CenterWell standards.
