Speech Therapist, Home Health

CenterWell Home Health is seeking a compassionate and skilled Speech Therapist to provide in-home speech-language pathology services to adult and senior patients. In this role, you will travel to patients’ homes to assess, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders, helping individuals maximize independence, safety, and quality of life in their own environment. You will conduct comprehensive evaluations, develop individualized plan-of-care treatment programs, and deliver evidence-based interventions aligned with physician orders, regulatory standards, and CenterWell’s commitment to high-quality care. You will collaborate closely with an interdisciplinary home health team—including nurses, physical and occupational therapists, social workers, and physicians—to coordinate care, participate in case conferences, update care plans, and ensure seamless communication regarding patient progress and changing needs. Documentation of assessments, goals, treatments, and outcomes will be completed accurately and timely in the electronic health record to support regulatory compliance and billing requirements. As a key member of a culture grounded in compassion, integrity, and excellence, you will educate patients and caregivers on home exercise programs, compensatory strategies, safe swallowing techniques, and communication supports, empowering families to participate actively in care. You will adhere strictly to infection control, safety protocols, and home health regulations while practicing independently in the field, organizing your schedule, managing your caseload, and prioritizing visits based on acuity and physician direction.
