Job responsibilities:

Perform medication reconciliation activities as directed and communicate discrepancies.
Support care coordination activities across providers to promote continuity of care.
Work 40 hours per week with 8-hour shifts.
Coordinate scheduling, referrals, and follow-up care with clinical and administrative teams.
